Inhibitors compete with other drugs for a particular enzyme thus reducing the rate of metabolism of the substrate drug which in many cases affects the individual's response to that particular medication, e.g. making it more toxic. Inducers increase the amount of enzyme thus increase the rate of metabolism of a substrate drug which in many cases affects the individual's response to a particular medication, for example making it ineffective.


Strong
A strong inhibitor causes a ≥ 5-fold increase in the area under the plasma concentration versus time curve (AUC) values or more than 80% decrease in clearance.

A strong inducer causes an ≥ 80% reduction in drug (substrate) exposure (AUC)



Moderate
A moderate inhibitor causes a ≥ 2-fold to < 5-fold increase in the plasma AUC values or 50-80% decrease in clearance.

A moderate inducer causes a 50% to < 80% reduction in drug (substrate) exposure (AUC)



Weak
A weak inhibitor causes a > 1.25-fold but < 2-fold increase in the plasma AUC values or 20-50% decrease in clearance.

A week inducer causes < 50% reduction in drug (substrate) exposure (AUC)
